Thrive 4 Peak Performance provides physician-supervised wellness and recovery services including IV therapy, cryotherapy, HOCATT, hyperbaric oxygen therapy, red light therapy, compression therapy, peptide therapy, and weight management programs.

2. Medical Disclaimer

Our services are not intended to diagnose, treat, cure, or prevent any disease. All treatments require medical screening and are provided under physician oversight. Individual results may vary.

3. Patient Responsibilities

By using our services, you agree to:

  • Provide accurate and complete medical history
  • Disclose all current medications and supplements
  • Follow pre- and post-treatment instructions
  • Inform us immediately of any adverse reactions
  • Attend scheduled appointments or cancel with 24-hour notice

4. Appointments and Cancellations

Cancellation Policy: We require 24-hour notice for appointment cancellations. Late cancellations or no-shows may be subject to a cancellation fee of up to 50% of the service cost.

5. Payment Terms

  • Payment is due at time of service unless other arrangements are made
  • We accept major credit cards, debit cards, and HSA/FSA cards
  • Package deals and memberships are non-refundable but transferable
  • Prices are subject to change with 30-day notice

6. Contraindications and Medical Clearance

Certain conditions may prevent you from receiving specific treatments. Our medical team will screen you before each service. You must obtain physician clearance if you have:

  • Pregnancy or suspected pregnancy
  • Heart conditions or pacemakers
  • Bleeding disorders or anticoagulant use
  • Active infections or fever
  • Recent surgeries (within 6 weeks)

Wellness therapies carry inherent risks. While we take every precaution, you acknowledge and accept these risks. You agree to release Thrive 4 Peak Performance, its physicians, and staff from liability for outcomes beyond our control.
